  • KWSP contribution statement, last 3 consecutive months, certified by KWSP
  • Audited accounts, or management accounts certified by an MIA-registered auditor
  • Bank statements, last 3 months, certified by the bank on page one of each month

And for plantation

  • Approval letter or Temporary Occupation Licence, where applicable

  • Birth certificate or marriage certificate, where applicable

Needed when the land worked is a parent's or a spouse's.

  • Tenancy or lease agreement of at least 3 years, stamped by LHDN

  • MPOB, Malaysian Rubber Board or Malaysian Cocoa Board certificate

    Whichever applies to the crop.

  • Land lot summary statement

The format is downloadable from the KDN website.
