The twelve documents. And where files actually die.

This is the document list from the Ministry of Home Affairs One Stop Centre checklist for the construction sector, dated 25 April 2025. It is public, it is free, and almost nobody reads it before they call an agent.

The requirements below are KDN’s, translated. The notes marked Where it fails are ours, and we have labelled them so you can tell the difference.

KDN’s words, on the form: Permohonan yang tidak lengkap, tidak teratur dan tidak menepati kriteria yang telah ditetapkan akan DITOLAK.

Applications that are incomplete, disorderly, or do not meet the criteria will be REJECTED.

Every applicant needs these eight

Dokumen Asas.

  1. 01

    The completed form, signed

    Signed by a director or the employer named in Borang 49 from SSM. Not by staff, not by your agent.

    Where it fails — our note

    If the signatory is not on Borang 49, the file goes back before anyone reads it.

  2. 02

    Section 60K approval certificate from JTKSM

    Sijil Kelulusan Penggajian Pekerja Asing Di Bawah Seksyen 60K Akta Kerja 1955.

    Where it fails — our note

    This is a separate application to a separate department, and you cannot start it the week you need workers. It is the single most common reason a contractor is months behind where he thinks he is.

  3. 03

    SSM company registration

    Borang 9, 13, 24 and 49, or Borang B and D — current, certified in the current year or printed from SSM, with the certification payment receipt attached.

    Where it fails — our note

    Certified last year does not count. Neither does a printout without the receipt.

  4. 04

    Owner or director's IC

    Copy, matching Borang 49 or Borang D.

  5. 05

    Company representative's credentials

    If someone attends for you: they must be executive or manager level and above, their name must appear in the complete KWSP statement for the last 3 consecutive months, plus their IC and a letter of authorisation signed by a director.

    Where it fails — our note

    You cannot send an admin clerk or your agent. If the person attending is not senior and not on your KWSP statements, the interview is wasted.

  6. 06

    KWSP contribution statement, 3 months

    The last 3 consecutive months, complete, certified by KWSP, with the certification payment receipt. Borang 'A' is explicitly not accepted.

    Where it fails — our note

    Certification by KWSP takes time and the form says Borang 'A' will not do. This is one of the two documents that most often stalls a file.

  7. 07

    Audited accounts

    Audited financial statements for the current or previous year. Sole proprietors, partnerships and enterprises may instead submit management accounts — balance sheet and profit and loss — certified true by an auditor registered with the Malaysian Institute of Accountants or an approved audit firm.

    “Certified by our accountant” is not the same as certified by an MIA-registered auditor.

  8. 08

    Bank statements, 3 months

    Company bank statements for the last 3 months, or the owner's savings or current account for sole proprietors, partnerships and enterprises. Statements printed online must be certified by the bank on the first page of every month.

    Every month needs its own bank certification. An online PDF on its own is rejected.

Construction adds four more

Dokumen Khusus — Sektor Pembinaan.

These four are why a construction application is not the same as a restaurant’s. Item 2 in particular decides whether you are eligible at all.

  1. 01

    Valid CIDB registration certificate

    Sijil perakuan pendaftaran CIDB that is still in force.

    Where it fails — our note

    Check the expiry date before you start. A certificate that lapses mid-process restarts you.

  2. 02

    Letter of Award or work contract, stamped by LHDN

    And the project must have at least 6 months remaining from its completion date at the time the application is submitted.

    Where it fails — our note

    This is the eligibility gate nobody mentions. If your project finishes in under six months, this application will not succeed no matter how clean everything else is — you need a longer project or a different one.

  3. 03

    CIMS CIDB project printout

    Printed project information as registered in the CIMS CIDB system, for the project being applied for only.

    Where it fails — our note

    The project has to already be registered in CIMS. If it is not, that is a job to do first.

  4. 04

    CIDB levy payment receipt

    For the project being carried out. Subcontractors must submit a copy of the main contractor's levy receipt.

    Where it fails — our note

    If you are the subcontractor, you need a document from the main contractor. Ask early — that request has been known to sit in somebody's inbox for weeks.

What else the form asks

And what you sign.

Beyond the documents, the form wants your CIDB grade and its expiry date, paid-up and authorised capital, your equity split, every current and upcoming project with its value and duration, your subcontractors, your entire existing workforce broken into seven job categories by local and foreign and by sex, the salary you are offering each requested worker, and where those workers will live.

The declaration you personally sign

Kementerian Dalam Negeri (KDN) dan Jabatan Imigresen Malaysia (JIM) berhak menyenarai hitam saya dan syarikat saya sekiranya didapati tidak mematuhi dasar dan syarat penggajian pekerja asing yang ditetapkan oleh kerajaan.

The Ministry of Home Affairs and the Immigration Department have the right to blacklist me and my company if I am found not to comply with the government's foreign worker employment policy and conditions.

This is the reason the file matters more than the agent. Whoever prepares it, the name on that declaration is yours, and the blacklist would be your company’s. Source, checked 15 August 2026.
